The Commonwealth Home Support Program assists individuals based on their disability or daily living skills. The services provided are daily meals, assistance with shopping, transport and domestic assistance.

SOCIAL GROUP ACTIVITIES

The Social Group Activities enable our clients to have interaction with other community elders, Reconnecting them to their past family and friends and avoids the Social isolation that frequently comes to the clients through lack of mobility, Sickness and lack of resources.
Staff members have access to arrange of activities services and will assist in increasing linkages, including local and regional networks and ensure clients have an input into the planning. Contribute to and conduct reviews, evaluations of services and maintain a culturally appropriate focus.
Our aim is supporting older people to become actively engaged in cultural activities within the local community will also impact on their well-being reducing their sense of isolation, dependence and physical wellbeing.

Access to receive Services is based on the assessment needs of each individuals seeking assistance to remain at home in the community pending their disability and daily living skills.
